Mobile Primary Care is a healthcare service that brings physicians directly to patients who are coping with chronic illness. These patients often experience severe pain and discomfort, making it difficult to complete even the simplest of daily tasks, including visiting a primary care physician. However, it is crucial for those with chronic illness to receive regular medical attention. With Mobile Primary Care, patients can receive the care they need in the comfort of their own homes, ensuring that they receive the necessary medical attention without the added stress of leaving their homes.

Most patients see their physicians in a traditional clinic setting where the patient comes to the doctor. However, since we all feel most comfortable at home, why not have our primary care physicians come into your home? That is exactly what you get when you receive primary care from Elevation Primary and Palliative — qualified, professional primary care provided directly within the walls of your own home.
Far too often, people let pain or anxiety stop them from seeking care from their primary care provider. This problem can be even worse for those facing severe, chronic illness. And it is for this purpose we bring the primary care directly to the patient’s home. If you or your loved one have difficulty going to see your doctor because of pain caused by severe illness, we want to provide primary care for them within the walls of their home.
Primary care can be provided in assisted living facilities, nursing homes, outpatient palliative care clinics, and other specialized clinics or in the comfort of your own home.
Finding care in assisted living facilities, nursing homes, or hospitals may be more complicated during COVID-19. However, these facilities may continue to update their services and policies to comply with state departments of Health and CDC guidelines to protect the health and safety of people receiving care and staff. Sage Palliative Care can help you navigate these uncertain times. To learn more about COVID-19 and the Sage Family of Companies practices click here.
Most private insurance plans cover some or all primary care services. Medicare and Medicaid also typically cover most of these services. Many insurance providers do require a small co-pay for primary care services. Veterans may also be eligible for primary care through the Department of Veterans Affairs. If you have concerns about paying for primary care, one of Elevation’s social workers or care managers will be glad to help you by calling (801) 810-2147.
